Floor 21 of Ultimate Challenge is the boss and the very final level of the game. This level starts after BJ acquires The Spear of Destiny at the end of Floor 18. On this level, which looks like a futuristic factory, BJ must confront the final boss, Devil Incarnate to prove once again that he is worth to wield the holy spear.

Trivia

If you look carefully, you will notice there is crate textures. These are the exact U.A.C crate textures from Doom. It is unknown if this was meant to be an Easter Egg, or an homage. The Ghosts are replaced by clouds of green radioactive mist, which is likewise a possible reference to the green radioactive slime in Doom. Another version of this map uses a different set of technology-themed textures without any Doom references.
